Uncertain significance for Developmental and epileptic encephalopathy, 8 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001353921.2(ARHGEF9):c.1193C>T (p.Ala398Val), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the ARHGEF9 gene (transcript NM_001353921.2) at coding-DNA position 1193, where C is replaced by T; at the protein level this means replaces alanine at residue 398 with valine — a missense variant. Submitter rationale: This sequence change replaces alanine with valine at codon 391 of the ARHGEF9 protein (p.Ala391Val). The alanine residue is highly conserved and there is a small physicochemical difference between alanine and valine. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has not been reported in the literature in individuals with ARHGEF9-related conditions. This variant is not present in population databases (ExAC no frequency).
