This variant lies in the ALG6 gene (transcript NM_013339.4) at coding-DNA position 1338, duplicating one base; at the protein level this means shifts the reading frame starting at valine residue 447, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: For these reasons, this variant has been classified as Pathogenic. Experimental studies and prediction algorithms are not available or were not evaluated, and the functional significance of this variant is currently unknown. ClinVar contains an entry for this variant (Variation ID: 851522). This premature translational stop signal has been observed in individual(s) with ALG6-congenital disorder of glycosylation (CDG-Ic) (PMID: 23430515, 27287710). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. This variant is not present in population databases (gnomAD no frequency). This sequence change creates a premature translational stop signal (p.Val447Serfs*44) in the ALG6 gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 61 amino acid(s) of the ALG6 protein.
