Uncertain significance for Leber congenital amaurosis 8; Retinitis pigmentosa 12 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_201253.3(CRB1):c.4169G>C (p.Arg1390Pro), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the CRB1 gene (transcript NM_201253.3) at coding-DNA position 4169, where G is replaced by C; at the protein level this means replaces arginine at residue 1390 with proline — a missense variant. Submitter rationale: This sequence change replaces arginine with proline at codon 1390 of the CRB1 protein (p.Arg1390Pro). The arginine residue is highly conserved and there is a moderate physicochemical difference between arginine and proline. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals affected with CRB1-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
