This variant lies in the NAGLU gene (transcript NM_000263.4) at coding-DNA position 1163 through coding-DNA position 1166, duplicating 4 bases; at the protein level this means shifts the reading frame starting at valine residue 390, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: For these reasons, this variant has been classified as Pathogenic. This variant disrupts the C-terminus of the NAGLU protein. Other variant(s) that disrupt this region (p.Trp404*) have been determined to be pathogenic (PMID: 9950362, 10094189). This suggests that variants that disrupt this region of the protein are likely to be causative of disease. This variant has not been reported in the literature in individuals with NAGLU-related conditions. This variant is not present in population databases (ExAC no frequency). This sequence change results in a premature translational stop signal in the NAGLU gene (p.Val390Alafs*72). While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 354 amino acids of the NAGLU protein.
