Uncertain significance for Rhabdoid tumor predisposition syndrome 2 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_003072.5(SMARCA4):c.758A>G (p.His253Arg), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces histidine with arginine at codon 253 of the SMARCA4 protein (p.His253Arg). The histidine residue is highly conserved and there is a small physicochemical difference between histidine and arginine. The frequency data for this variant in the population databases is considered unreliable, as metrics indicate insufficient coverage at this position in the ExAC database. This variant has not been reported in the literature in individuals with SMARCA4-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain.
