NM_001100.4(ACTA1):c.842A>G (p.Tyr281Cys) was classified as Pathogenic for Actin accumulation myopathy by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015). This variant lies in the ACTA1 gene (transcript NM_001100.4) at coding-DNA position 842, where A is replaced by G; at the protein level this means replaces tyrosine at residue 281 with cysteine — a missense variant. Submitter rationale: This sequence change replaces tyrosine, which is neutral and polar, with cysteine, which is neutral and slightly polar, at codon 281 of the ACTA1 protein (p.Tyr281Cys). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with congenital myopathy and/or nemaline myopathy (internal data). In at least one individual the variant was observed to be de novo. It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 848617). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt ACTA1 protein function with a negative predictive value of 80%. This variant disrupts the p.Tyr281 amino acid residue in ACTA1. Other variant(s) that disrupt this residue have been observed in individuals with ACTA1-related conditions (PMID: 12921789), which suggests that this may be a clinically significant amino acid residue. For these reasons, this variant has been classified as Pathogenic.
