NM_000383.4(AIRE):c.607C>T (p.Arg203Ter) was classified as Pathogenic for Polyglandular autoimmune syndrome, type 1 by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015). This variant lies in the AIRE gene (transcript NM_000383.4) at coding-DNA position 607, where C is replaced by T; at the protein level this means converts the codon for arginine at residue 203 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: This sequence change creates a premature translational stop signal (p.Arg203*) in the AIRE gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in AIRE are known to be pathogenic (PMID: 11524731, 26141571). This variant is present in population databases (rs755490967, gnomAD 0.003%). This premature translational stop signal has been observed in individual(s) with autosomal recessive autoimmune polyendocrinopathy syndrome (PMID: 9717837, 10677297, 12050215, 18274776, 22024611). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 848152). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. For these reasons, this variant has been classified as Pathogenic.
