Pathogenic for Hypertrophic cardiomyopathy — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000257.4(MYH7):c.2146G>C (p.Gly716Arg), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the MYH7 gene (transcript NM_000257.4) at coding-DNA position 2146, where G is replaced by C; at the protein level this means replaces glycine at residue 716 with arginine — a missense variant. Submitter rationale: This variant is found within a region of MYH7 between codons 181 and 937 that contains the majority of the myosin head domain. Missense variants in this region have been shown to be significantly overrepresented in individuals with hypertrophic cardiomyopathy (PMID: 27532257). For these reasons, this variant has been classified as Pathogenic. A different variant (c.2146G>A) giving rise to the same protein effect observed here (p.Gly716Arg) has been determined to be pathogenic (PMID: 25935763, 8282798,20624503, 23283745, 12707239, 20800588, 27161882, 30165862). This suggests that this variant is also likely to be causative of disease. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This missense change has been observed in individuals affected with hypertrophic cardiomyopathy (PMID: 27483260, 28356264, 15358028, 18953637, 24093860, 20031618). This variant is not present in population databases (ExAC no frequency). This sequence change replaces glycine with arginine at codon 716 of the MYH7 protein (p.Gly716Arg). The glycine residue is highly conserved and there is a moderate physicochemical difference between glycine and arginine.
