Pathogenic for Bardet-Biedl syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NC_000002.12:g.169504481_169504487del, citing Invitae Variant Classification Sherloc (09022015): This sequence change creates a premature translational stop signal (p.Gln309Ilefs*14) in the BBS5 gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 33 amino acid(s) of the BBS5 protein. This variant is not present in population databases (gnomAD no frequency). This premature translational stop signal has been observed in individual(s) with clinical features of Bardet-Biedl syndrome (PMID: 32811249). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 846188). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. This variant disrupts a region of the BBS5 protein in which other variant(s) ( p.Glu319del) have been observed in individuals with BBS5-related conditions (PMID: 26355662). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic.
