Uncertain significance for Progressive myoclonic epilepsy — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_005670.4(EPM2A):c.505T>C (p.Cys169Arg), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the EPM2A gene (transcript NM_005670.4) at coding-DNA position 505, where T is replaced by C; at the protein level this means replaces cysteine at residue 169 with arginine — a missense variant. Submitter rationale: This sequence change replaces cysteine with arginine at codon 169 of the EPM2A protein (p.Cys169Arg). The cysteine residue is highly conserved and there is a large physicochemical difference between cysteine and arginine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with EPM2A-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
