Uncertain significance for Cryopyrin associated periodic syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001243133.2(NLRP3):c.442T>G (p.Cys148Gly), citing Invitae Variant Classification Sherloc (09022015): In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. This sequence change replaces cysteine with glycine at codon 150 of the NLRP3 protein (p.Cys150Gly). The cysteine residue is moderately conserved and there is a large physicochemical difference between cysteine and glycine. This variant has not been reported in the literature in individuals with NLRP3-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant is not present in population databases (ExAC no frequency).
