Pathogenic for Early-infantile DEE — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_172107.4(KCNQ2):c.667T>C (p.Ser223Pro), citing Invitae Variant Classification Sherloc (09022015): For these reasons, this variant has been classified as Pathogenic. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has been observed in individual(s) with clinical features of early-onset epileptic encephalopathy (PMID: 28133863, Invitae). In at least one individual the variant was observed to be de novo. This variant is not present in population databases (ExAC no frequency). This sequence change replaces serine with proline at codon 223 of the KCNQ2 protein (p.Ser223Pro). The serine residue is highly conserved and there is a moderate physicochemical difference between serine and proline.
