Uncertain significance for Pyridoxine-dependent epilepsy — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001182.5(ALDH7A1):c.1235C>T (p.Thr412Ile), citing Invitae Variant Classification Sherloc (09022015): Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. This variant has not been reported in the literature in individuals with ALDH7A1-related conditions. This variant is not present in population databases (ExAC no frequency). This sequence change replaces threonine with isoleucine at codon 412 of the ALDH7A1 protein (p.Thr412Ile). The threonine residue is highly conserved and there is a moderate physicochemical difference between threonine and isoleucine.
