Uncertain significance for Neuronopathy, distal hereditary motor, type 7A; Congenital myasthenic syndrome 20 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_021815.5(SLC5A7):c.121G>A (p.Ala41Thr), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the SLC5A7 gene (transcript NM_021815.5) at coding-DNA position 121, where G is replaced by A; at the protein level this means replaces alanine at residue 41 with threonine — a missense variant. Submitter rationale: Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. This variant has not been reported in the literature in individuals with SLC5A7-related conditions. This variant is not present in population databases (ExAC no frequency). This sequence change replaces alanine with threonine at codon 41 of the SLC5A7 protein (p.Ala41Thr). The alanine residue is weakly conserved and there is a small physicochemical difference between alanine and threonine.
