Uncertain significance for Primary ciliary dyskinesia 23 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_018076.5(ODAD2):c.1345T>G (p.Ser449Ala), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the ODAD2 gene (transcript NM_018076.5) at coding-DNA position 1345, where T is replaced by G; at the protein level this means replaces serine at residue 449 with alanine — a missense variant. Submitter rationale: This sequence change replaces serine with alanine at codon 449 of the ARMC4 protein (p.Ser449Ala). The serine residue is moderately conserved and there is a moderate physicochemical difference between serine and alanine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with ARMC4-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
