Pathogenic for Mucopolysaccharidosis type 1 — the classification assigned by ClinGen Lysosomal Storage Disorder Variant Curation Expert Panel to NM_000203.5(IDUA):c.1163del (p.Thr388fs), citing ClinGen LSD ACMG Specifications IDUA V1.0.0. This variant lies in the IDUA gene (transcript NM_000203.5) at coding-DNA position 1163, deleting one base; at the protein level this means shifts the reading frame starting at threonine residue 388, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: The NM_000203.5(IDUA):c.1163delC (p.Thr388ArgfsTer52) variant in IDUA is a frameshift variant predicted to cause a premature stop codon leading to nonsense mediated decay in a gene in which loss-of-function is an established disease mechanism (PVS1). Two Italian patients with this variant and severe MPS1 have been reported. The diagnosis was based on clinical features, elevated urine GAGs, and reduced IDUA activity in fibroblasts and/or leukocyte. However, individual data were not provided. One patient was compound heterozygous for the variant and another variant in IDUA that has been classified as pathogenic by the ClinGen Lysosomal Diseases VCEP, c.1205G>A (p.Trp402Ter); the phase is unknown (PMID: 12203999). The other patient was compound heterozygous for the variant and c.152G>A (p.Gly51Asp) (PMID: 12203999); the data from the patient will be used in the assessment of p.G;y51Asp and was not included here to avoid circular logic.
