Likely pathogenic for STAT3 gain of function; Hyper-IgE recurrent infection syndrome 1, autosomal dominant — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_139276.3(STAT3):c.870A>T (p.Lys290Asn), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces lysine, which is basic and polar, with asparagine, which is neutral and polar, at codon 290 of the STAT3 protein (p.Lys290Asn). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of STAT3-related conditions (PMID: 29180260). In at least one individual the variant was observed to be de novo. ClinVar contains an entry for this variant (Variation ID: 827746). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt STAT3 protein function with a negative predictive value of 80%. Experimental studies have shown that this missense change affects STAT3 function (PMID: 29180260). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
