Pathogenic — the classification assigned by GeneDx to NM_015100.4(POGZ):c.2164dup (p.Met722fs), citing GeneDx Variant Classification (06012015). This variant lies in the POGZ gene (transcript NM_015100.4) at coding-DNA position 2164, duplicating one base; at the protein level this means shifts the reading frame starting at methionine residue 722, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: The c.2164dupA variant in the POGZ gene has not been reported previously as a pathogenic variant nor as a benign variant, to our knowledge. The c.2164dupA variant causes a frameshift starting with codon Methionine 722, changes this amino acid to an Asparagine residue, and creates a premature Stop codon at position 22 of the new reading frame, denoted p.Met722AsnfsX22. This variant is predicted to cause loss of normal protein function either through protein truncation or nonsense-mediated mRNA decay. The c.2164dupA variant is not observed in large population cohorts (Lek et al., 2016). We interpret c.2164dupA as a pathogenic variant.
