Pathogenic — the classification assigned by GeneDx to NM_001378454.1(ALMS1):c.2617C>T (p.Gln873Ter), citing GeneDx Variant Classification (06012015). This variant lies in the ALMS1 gene (transcript NM_001378454.1) at coding-DNA position 2617, where C is replaced by T; at the protein level this means converts the codon for glutamine at residue 873 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: The Q874X variant in the ALMS1 gene has not been reported previously as a pathogenic variant nor as a benign variant, to our knowledge. This variant is predicted to cause loss of normal protein function either through protein truncation or nonsense-mediated mRNA decay. The Q874X variant is not observed in large population cohorts (Lek et al., 2016). We interpret Q874X as a pathogenic variant.
