NM_001384732.1(CPLANE1):c.3507+1G>A was classified as Pathogenic for Joubert syndrome 17 by Broad Center for Mendelian Genomics, Broad Institute of MIT and Harvard, citing ACMG Guidelines, 2015. This variant lies in the CPLANE1 gene (transcript NM_001384732.1) at the canonical splice donor site of the intron immediately after coding-DNA position 3507, where G is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The heterozygous c.3507+1G>A variant in C5orf42 was identified by our study in the compound heterozygous state, with another pathogenic variant, in one individual with Joubert syndrome. The presence of this variant in combination with a pathogenic variant and in an individual with Joubert syndrome increases the likelihood that the c.3507+1G>A variant is pathogenic. The variant was absent from large population studies. The c.3507+1G>A variant occurs in the invariant region (+/- 1/2) of the splice consensus sequence and is predicted to cause altered splicing leading to an abnormal or absent protein. Loss of function of the C5orf42 gene is an established disease mechanism in autosomal recessive Joubert syndrome, and this is a loss of function variant. In summary, this variant meets criteria to be classified as pathogenic for Joubert syndrome in an autosomal recessive manner based on the predicted impact of the variant and the presence of a pathogenic variant in trans in an individual with Joubert syndrome.
