Pathogenic for T-B+ severe combined immunodeficiency due to JAK3 deficiency — the classification assigned by ClinGen Severe Combined Immunodeficiency Variant Curation Expert Panel, ClinGen to NM_000215.4(JAK3):c.1351C>T (p.Arg451Ter), citing ClinGen SCID ACMG Specifications JAK3 V1.0.0: The c.1351C>T (p.Arg451Ter)(NM_000215.4) variant in JAK3 is a nonsense variant predicted to cause a premature stop codon in biologically relevant exon 10/24 leading to nonsense-mediated decay in a gene in which loss-of-function is an established disease mechanism (PVS1). The highest population minor allele frequency in gnomAD v2.1.1 is 0.0001150 (1/8692 alleles) in the African/African American population, which is the same as the ClinGen SCID VCEP threshold (<0.0001150 ) for PM2_supporting because this was the variant used to set our PM2 threshold (The most frequently pathogenic variant following SCID VCEP specifications). As the variant is pretty rare in genomes (1/8692) and is not found in exomes PM2 was applied at the supporting level of evidence. Two patients were reported in the literature: * Patient 8: Diagnostic criteria for SCID/Leaky SCID/Omenn syndrome met (0.5pt), SCID gene panel or exome/genome sequencing conducted (0.5pt), T-B+NK- lymphocyte subset profile (0.5pt), total 1.5pt, PP4_Supporting. PMID: 33040328. * Patient 54: Diagnostic criteria for SCID/Leaky SCID/Omenn syndrome met (0.5pt), SCID gene panel or exome/genome sequencing conducted (0.5pt), T-B+NK- lymphocyte subset profile (0.5pt), total 1.5pt, PP4_Supporting. PMID: 30778343. Both of them were homozygous for the variant: 0.5 pts each, 1 pt total, PM3_moderate. (Patient 8, PMID: 33040328 and Patient 54, PMID: 30778343). Two additional patients were reported on ClinVar with no genetic information available; Confirmed affected status just for one of them. In summary, this variant meets the criteria to be classified as Pathogenic for autosomal recessive SCID based on the ACMG/AMP criteria applied, as specified by the ClinGen SCID VCEP. Criteria applied: PVS1, PM3, PP4, and PM2_Supporting. (VCEP specifications version 1).
