Uncertain significance for Leber congenital amaurosis 6; Cone-rod dystrophy 13 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_020366.4(RPGRIP1):c.2468A>G (p.Tyr823Cys), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the RPGRIP1 gene (transcript NM_020366.4) at coding-DNA position 2468, where A is replaced by G; at the protein level this means replaces tyrosine at residue 823 with cysteine — a missense variant. Submitter rationale: This sequence change replaces tyrosine, which is neutral and polar, with cysteine, which is neutral and slightly polar, at codon 823 of the RPGRIP1 protein (p.Tyr823Cys). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with Leber congenital amaurosis (PMID: 32865313). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 803004). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt RPGRIP1 protein function with a positive predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
