This variant lies in the MSH2 gene (transcript NM_000251.3) at the canonical splice acceptor site of the intron immediately before coding-DNA position 2211, where G is replaced by C; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.2211-1G>C intronic variant results from a G to C substitution one nucleotide upstream before coding exon 14 of the MSH2 gene. This variant has been identified in a proband(s) whose Lynch syndrome-associated tumor demonstrated high microsatellite instability and loss of MSH2/MSH6 expression by immunohistochemistry (Ambry internal data). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice acceptor site; however, direct evidence is insufficient at this time (Ambry internal data). Alterations that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. As such, this alteration is classified as likely pathogenic.
