Likely Benign for Open-angle glaucoma — the classification assigned by ClinGen Glaucoma Variant Curation Expert Panel to NM_000261.2(MYOC):c.136C>T (p.Arg46Ter), citing ClinGen Glaucoma ACMG Specifications V2.0.0 Approved. This variant lies in the MYOC gene (transcript NM_000261.2) at coding-DNA position 136, where C is replaced by T; at the protein level this means converts the codon for arginine at residue 46 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: The c.136C>T variant in MYOC is predicted to cause a change in the length of the protein due to the insertion of a terminating codon instead of the usual Arginine at amino acid 46 (p.Arg46Ter). Truncation of this protein occurs outside of the conserved olfactomedin domain, which did not meet PM4. PVS1 did not apply, as the disease mechanism for MYOC variants associated with juvenile or primary open angle glaucoma (JOAG or POAG) is not loss-of-function. The highest minor allele frequency of this variant was in the East Asian genetic ancestry group of gnomAD (v4.1.0) = 0.006416, which met the ≥ 0.001 threshold set for BS1 (288 alleles out of 44,888), meeting the threshold of ≥ 5 of at least 2,000 observed alleles). There was no functional evidence predicting a damaging or benign impact of this variant on MYOC function. As BS1 was met, PP1 did not apply and segregations were not counted. Although probands with JOAG or POAG have been reported carrying this variant, PM2_Supporting was not met, therefore PS4 did not apply.
