Pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_153766.3(KCNJ1):c.504del (p.Lys168fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the KCNJ1 gene (transcript NM_153766.3) at coding-DNA position 504, deleting one base; at the protein level this means shifts the reading frame starting at lysine residue 168, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This sequence change creates a premature translational stop signal (p.Lys187Asnfs*13) in the KCNJ1 gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 205 amino acid(s) of the KCNJ1 protein. For these reasons, this variant has been classified as Pathogenic. This variant disrupts a region of the KCNJ1 protein in which other variant(s) (p.His354Serfs*8) have been determined to be pathogenic (PMID: 11318951). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. ClinVar contains an entry for this variant (Variation ID: 694393). This variant has not been reported in the literature in individuals affected with KCNJ1-related conditions. This variant is not present in population databases (gnomAD no frequency).
