Likely pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_002715.4(PPP2CA):c.380A>G (p.Tyr127Cys), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces tyrosine, which is neutral and polar, with cysteine, which is neutral and slightly polar, at codon 127 of the PPP2CA protein (p.Tyr127Cys). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with intellectual disability (PMID: 30595372). In at least one individual the variant was observed to be de novo. ClinVar contains an entry for this variant (Variation ID: 692148). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt PPP2CA protein function with a positive predictive value of 80%. Experimental studies have shown that this missense change affects PPP2CA function (PMID: 30595372). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
