Pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_139027.6(ADAMTS13):c.1045C>T (p.Arg349Cys), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces arginine, which is basic and polar, with cysteine, which is neutral and slightly polar, at codon 349 of the ADAMTS13 protein (p.Arg349Cys). For these reasons, this variant has been classified as Pathogenic. Experimental studies have shown that this missense change affects ADAMTS13 function (PMID: 29554699, 32365113). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt ADAMTS13 protein function. ClinVar contains an entry for this variant (Variation ID: 68799). This missense change has been observed in individual(s) with thrombotic thrombocytopenic purpura (PMID: 19055667, 29554699). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the gnomAD database.
