NM_004990.4(MARS1):c.1568T>C (p.Ile523Thr) was classified as Uncertain significance for Severe early-onset pulmonary alveolar proteinosis due to MARS deficiency; Charcot-Marie-Tooth disease axonal type 2U by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces isoleucine, which is neutral and non-polar, with threonine, which is neutral and polar, at codon 523 of the MARS protein (p.Ile523Thr). This variant is present in population databases (rs201555303, gnomAD 0.03%). This missense change has been observed in individual(s) with clinical features of autosomal recessive interstitial lung and liver disease (PMID: 24103465). ClinVar contains an entry for this variant (Variation ID: 68464). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Experimental studies have shown that this missense change affects MARS function (PMID: 24103465). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
