Uncertain significance for Developmental and epileptic encephalopathy, 42; Episodic ataxia type 2 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001127222.2(CACNA1A):c.4474G>A (p.Val1492Ile), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces valine, which is neutral and non-polar, with isoleucine, which is neutral and non-polar, at codon 1493 of the CACNA1A protein (p.Val1493Ile). This variant is present in population databases (rs121908234, gnomAD 0.004%). This missense change has been observed in individual(s) with episodic ataxia type 2 (PMID: 15173248). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 68431). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on CACNA1A protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
