This variant lies in the ARSA gene (transcript NM_000487.6) at coding-DNA position 640, where G is replaced by C; at the protein level this means replaces alanine at residue 214 with proline — a missense variant. Submitter rationale: Variant summary: ARSA c.640G>C (p.Ala214Pro) results in a non-conservative amino acid change in the encoded protein sequence. Three of four in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 250630 control chromosomes. c.640G>C has been reported in the literature in multiple individuals affected with Metachromatic Leukodystrophy (e.g., Biffi_2008, Beerepoot_2020, Pekgul_2020). These data indicate that the variant is very likely to be associated with disease. A different variant affecting the same codon has been classified as pathogenic by our lab (c.641C>T, p.Ala214Val), supporting the critical relevance of codon 214 to ARSA protein function. At least one publication reports experimental evidence evaluating an impact on protein function. The most pronounced variant effect results in <1% of normal activity (e.g., Cesari_2009).
