NM_002485.5(NBN):c.1770A>T (p.Arg590Ser) was classified as Uncertain significance for Hereditary cancer-predisposing syndrome by Ambry Genetics, citing Ambry Variant Classification Scheme 2023. This variant lies in the NBN gene (transcript NM_002485.5) at coding-DNA position 1770, where A is replaced by T; at the protein level this means replaces arginine at residue 590 with serine — a missense variant. Submitter rationale: The p.R590S variant (also known as c.1770A>T), located in coding exon 11 of the NBN gene, results from an A to T substitution at nucleotide position 1770. The arginine at codon 590 is replaced by serine, an amino acid with dissimilar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6502 samples (13004 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.001% (greater than 120000 alleles tested) in our clinical cohort. This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of p.R590S remains unclear.
