Uncertain significance for Cardiovascular phenotype — the classification assigned by Ambry Genetics to NM_001148.6(ANK2):c.2890A>G (p.Ile964Val), citing Ambry Variant Classification Scheme 2023: The p.I964V variant (also known as c.2890A>G), located in coding exon 26 of the ANK2 gene, results from an A to G substitution at nucleotide position 2890. The isoleucine at codon 964 is replaced by valine, an amino acid with highly similar properties. This variant was detected in an individual from a hospital-based cohort with prolonged QTc interval who may have had a non-genetic explanation for QTc prolongation (Gibbs C et al. J Am Heart Assoc, 2018 Aug;7:e009706). This variant was also reported to co-occur with a variant in the DSP gene in an individual from an arrhythmogenic right ventricular cardiomyopathy cohort (Roberts JD et al. J. Clin. Invest., 2019 07;129:3171-3184). This amino acid position is not well conserved in available vertebrate species, and valine is the reference amino acid in other vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
