Uncertain significance for Mevalonic aciduria; Hyperimmunoglobulin D with periodic fever; Porokeratosis 3, disseminated superficial actinic type — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000431.4(MVK):c.166A>G (p.Ile56Val), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces isoleucine with valine at codon 56 of the MVK protein (p.Ile56Val). The isoleucine residue is moderately conserved and there is a small physicochemical difference between isoleucine and valine. This variant is present in population databases (rs149095003, ExAC 0.003%). This missense change has been observed in individual(s) with recurrent fever episodes (PMID: 26386126). Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
