Uncertain significance for Arthrogryposis, distal, type 1A — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_003289.4(TPM2):c.533G>T (p.Arg178Leu), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the TPM2 gene (transcript NM_003289.4) at coding-DNA position 533, where G is replaced by T; at the protein level this means replaces arginine at residue 178 with leucine — a missense variant. Submitter rationale: This sequence change replaces arginine with leucine at codon 178 of the TPM2 protein (p.Arg178Leu). The arginine residue is highly conserved and there is a moderate physicochemical difference between arginine and leucine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with TPM2-related disease. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
