Uncertain significance for Epileptic encephalopathy — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001036.6(RYR3):c.3800C>T (p.Pro1267Leu), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the RYR3 gene (transcript NM_001036.6) at coding-DNA position 3800, where C is replaced by T; at the protein level this means replaces proline at residue 1267 with leucine — a missense variant. Submitter rationale: This sequence change replaces proline with leucine at codon 1267 of the RYR3 protein (p.Pro1267Leu). The proline residue is highly conserved and there is a moderate physicochemical difference between proline and leucine. This variant is present in population databases (rs768607761, ExAC 0.009%). This variant has not been reported in the literature in individuals with RYR3-related disease. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
