Pathogenic variants in this gene have been associated with autosomal recessive cerebrotendinous xanthomatosis. This variant introduces a premature termination codon in exon 4 out of 9 and is expected to result in loss of function, which is a known disease mechanism for CYP27A1 in this disorder (PMID: 9548584, 26937392) (PVS1). This variant has been identified in the compound heterozygous state in at least one individual reported in the published literature (PMID: 9392430) (PM3), and it has a 0.0022% maximum allele frequency in non-founder control populations (https://gnomad.broadinstitute.org/) (PM2). Based on the current evidence, this variant is classified as pathogenic for autosomal recessive cerebrotendinous xanthomatosis.
