Pathogenic for Deficiency of isobutyryl-CoA dehydrogenase — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_014384.3(ACAD8):c.1000C>T (p.Arg334Cys), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces arginine, which is basic and polar, with cysteine, which is neutral and slightly polar, at codon 334 of the ACAD8 protein (p.Arg334Cys). This variant is present in population databases (rs778823613, gnomAD 0.03%). This missense change has been observed in individual(s) with isobutyryl CoA dehydrogenase deficiency (PMID: 16857760, 17924841, 30253142, 35154245). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 658754). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt ACAD8 protein function with a positive predictive value of 80%.
