Uncertain significance for Microcephaly-intellectual disability-sensorineural hearing loss-epilepsy-abnormal muscle tone syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_145207.3(AFG2A):c.410C>T (p.Ala137Val), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the AFG2A gene (transcript NM_145207.3) at coding-DNA position 410, where C is replaced by T; at the protein level this means replaces alanine at residue 137 with valine — a missense variant. Submitter rationale: In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has not been reported in the literature in individuals with SPATA5-related disease. This variant is present in population databases (rs778362935, ExAC 0.006%). This sequence change replaces alanine with valine at codon 137 of the SPATA5 protein (p.Ala137Val). The alanine residue is highly conserved and there is a small physicochemical difference between alanine and valine.
