Pathogenic for Early-infantile DEE — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001165963.4(SCN1A):c.4852+1G>T, citing Invitae Variant Classification Sherloc (09022015). This variant lies in the SCN1A gene (transcript NM_001165963.4) at the canonical splice donor site of the intron immediately after coding-DNA position 4852, where G is replaced by T; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: For these reasons, this variant has been classified as Pathogenic. This sequence change affects a donor splice site in the last intron (intron 25) of the SCN1A gene. While this is not anticipated to result in nonsense mediated decay, it likely alters RNA splicing and results in a disrupted protein product. This variant is not present in population databases (ExAC no frequency). This variant has been observed in individual(s) with clinical features of generalized epilepsy with febrile seizures plus (GEFS+) and/or Dravet syndrome (PMID: 28202706, 21248271, Invitae). In at least one individual the variant was observed to be de novo. ClinVar contains an entry for this variant (Variation ID: 653218). Nucleotide substitutions within the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site, but this prediction has not been confirmed by published transcriptional studies.
