NM_000222.3(KIT):c.1526A>T (p.Lys509Ile) was classified as Pathogenic for Gastrointestinal stromal tumor by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015). This variant lies in the KIT gene (transcript NM_000222.3) at coding-DNA position 1526, where A is replaced by T; at the protein level this means replaces lysine at residue 509 with isoleucine — a missense variant. Submitter rationale: This sequence change replaces lysine with isoleucine at codon 509 of the KIT protein (p.Lys509Ile). The lysine residue is highly conserved and there is a moderate physicochemical difference between lysine and isoleucine. This variant is not present in population databases (ExAC no frequency). This variant has been observed to be de novo in several individuals affected with mastocytosis (PMID: 25139846, 24582309, 23610110). One of these individuals was also affected with gastrointestinal stromal tumor (GIST) (PMID: 23610110). It has been observed to segregate with disease in at least two families (PMID: 16183119, 25139846). Experimental studies have shown that this missense change increases mast cell progenitor expansion and shows ligand-independent cell survival in vitro (PMID: 24582309). In some studies, patient-derived cells carrying this variant respond to the kinase inhibitor treatment in vitro (PMID: 16183119, 25139846, 29439183). For these reasons, this variant has been classified as Pathogenic.
