Uncertain significance for Familial acute necrotizing encephalopathy — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_006267.5(RANBP2):c.6857A>G (p.Gln2286Arg), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the RANBP2 gene (transcript NM_006267.5) at coding-DNA position 6857, where A is replaced by G; at the protein level this means replaces glutamine at residue 2286 with arginine — a missense variant. Submitter rationale: Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. This variant has not been reported in the literature in individuals with RANBP2-related disease. This variant is not present in population databases (ExAC no frequency). This sequence change replaces glutamine with arginine at codon 2286 of the RANBP2 protein (p.Gln2286Arg). The glutamine residue is highly conserved and there is a small physicochemical difference between glutamine and arginine.
