Pathogenic for Marfan syndrome; Familial thoracic aortic aneurysm and aortic dissection — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000138.5(FBN1):c.2053T>A (p.Cys685Ser), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the FBN1 gene (transcript NM_000138.5) at coding-DNA position 2053, where T is replaced by A; at the protein level this means replaces cysteine at residue 685 with serine — a missense variant. Submitter rationale: This sequence change replaces cysteine with serine at codon 685 of the FBN1 protein (p.Cys685Ser). The cysteine residue is highly conserved and there is a moderate physicochemical difference between cysteine and serine. This variant is not present in population databases (ExAC no frequency). This variant has been observed in an individual affected with clinical features of FBN1-related disease (Invitae). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site, but this prediction has not been confirmed by published transcriptional studies. This variant disrupts the p.Cys685 amino acid residue in FBN1. Other variant(s) that disrupt this residue have been observed in affected individuals (PMID: 21542060, 12203992, 15241795), which suggests that this may be a clinically significant amino acid residue. For these reasons, this variant has been classified as Pathogenic. This variant affects a cysteine residue in the EGF-like, TGFBP or hybrid motif domains FBN1. Cysteine residues are believed to be involved in intramolecular disulfide bridges and have been shown to be important for FBN1 protein structure (PMID: 16905551, 19349279). In addition, missense substitutions affecting cysteine residues within these domains are significantly overrepresented among patients with Marfan syndrome (PMID: 16571647, 17701892).
