Pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000179.3(MSH6):c.313del (p.Trp105fs), citing Ambry Variant Classification Scheme 2023. This variant lies in the MSH6 gene (transcript NM_000179.3) at coding-DNA position 313, deleting one base; at the protein level this means shifts the reading frame starting at tryptophan residue 105, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: The c.313delT pathogenic mutation, located in coding exon 2 of the MSH6 gene, results from a deletion of one nucleotide at nucleotide position 313, causing a translational frameshift with a predicted alternate stop codon (p.W105Gfs*44). This variant has been observed in at least one individual with a personal and/or family history that is consistent with Lynch syndrome (Ambry internal data). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation.
