Uncertain significance for Febrile seizures, familial, 8; EPILEPSY, CHILDHOOD ABSENCE, SUSCEPTIBILITY TO, 2 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_198904.4(GABRG2):c.106G>A (p.Gly36Ser), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the GABRG2 gene (transcript NM_198904.4) at coding-DNA position 106, where G is replaced by A; at the protein level this means replaces glycine at residue 36 with serine — a missense variant. Submitter rationale: This sequence change replaces glycine with serine at codon 36 of the GABRG2 protein (p.Gly36Ser). The glycine residue is moderately conserved and there is a small physicochemical difference between glycine and serine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with GABRG2-related disease. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site, but this prediction has not been confirmed by published transcriptional studies. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
