Uncertain significance for Polymerase proofreading-related adenomatous polyposis — the classification assigned by Department of Pathology and Laboratory Medicine, Sinai Health System to NM_006231.4(POLE):c.43G>C (p.Ala15Pro). This variant lies in the POLE gene (transcript NM_006231.4) at coding-DNA position 43, where G is replaced by C; at the protein level this means replaces alanine at residue 15 with proline — a missense variant. Submitter rationale: The POLE p.Ala15Pro variant was not identified in the literature nor was it identified in the dbSNP and ClinVar databases. The variant was not identified in the following control databases: the Exome Aggregation Consortium (August 8th 2016), or the Genome Aggregation Database (Feb 27, 2017). The variant was identified by our laboratory in 1 individual with colorectal adenomas, co-occurring with a pathogenic APC variant (c.994C>T, p.Arg332X) increasing the likelihood the variant does not have clinical significance. The p.Ala15 residue is conserved in mammals and computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) provide inconsistent predictions regarding the impact of the variant to the protein; this information is not very predictive of pathogenicity. The variant occurs outside of the splicing consensus sequence and 1 of 4 in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer) predict a greater than 10% difference in splicing; this is not very predictive of pathogenicity. In summary, based on the above information the clinical significance of this variant cannot be determined with certainty at this time.
