Uncertain significance for Neuronopathy, distal hereditary motor, autosomal recessive 4; Charcot-Marie-Tooth disease recessive intermediate C — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_020631.6(PLEKHG5):c.1139C>T (p.Ala380Val), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the PLEKHG5 gene (transcript NM_020631.6) at coding-DNA position 1139, where C is replaced by T; at the protein level this means replaces alanine at residue 380 with valine — a missense variant. Submitter rationale: This sequence change replaces alanine with valine at codon 380 of the PLEKHG5 protein (p.Ala380Val). The alanine residue is moderately conserved and there is a small physicochemical difference between alanine and valine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals affected with PLEKHG5-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function output the following: SIFT: "Tolerated"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0". The valine amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
