Uncertain significance for Combined immunodeficiency due to LRBA deficiency — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001364905.1(LRBA):c.3437A>T (p.Asp1146Val), citing Invitae Variant Classification Sherloc (09022015): This variant has not been reported in the literature in individuals with LRBA-related disease. This sequence change replaces aspartic acid with valine at codon 1146 of the LRBA protein (p.Asp1146Val). The aspartic acid residue is weakly conserved and there is a large physicochemical difference between aspartic acid and valine. This variant is not present in population databases (ExAC no frequency). Algorithms developed to predict the effect of missense changes on protein structure and function output the following: SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0". The valine amino acid residue is found in multiple mammalian species, suggesting that this missense change does not adversely affect protein function. These predictions have not been confirmed by published functional studies and their clinical significance is uncertain.
