Uncertain significance for Abnormality of the nervous system; Hereditary spastic paraplegia 48 — the classification assigned by Neuberg Centre For Genomic Medicine, NCGM to NM_014855.3(AP5Z1):c.1072C>T (p.Arg358Cys), citing ACMG Guidelines, 2015. This variant lies in the AP5Z1 gene (transcript NM_014855.3) at coding-DNA position 1072, where C is replaced by T; at the protein level this means replaces arginine at residue 358 with cysteine — a missense variant. Submitter rationale: The observed missense variant c.1072C>T(p.Arg358Cys) in the AP5Z1 gene has not been reported previously as a pathogenic variant nor as a benign variant, to our knowledge. This variant is reported with the allele frequency 0.02% in the gnomAD Exomes. This variant has been reported to the ClinVar database as Uncertain Significance. However study of the variant in multiple affected individuals and its functional impact on the protein is required to determine the pathogenicity of the variant. The amino acid Arginine at position 358 is changed to a Cysteine changing protein sequence and it might alter its composition and physico-chemical properties. Multiple lines of computational evidence (Polyphen - Benign, SIFT - Tolerated and MutationTaster - Polymorphism) predict no damaging effect on protein structure and function for this variant. The residue is conserved by GERP++ and PhyloP across 100 vertebrates. For these reasons, this variant has been classified as Uncertain Significance.
