Pathogenic for Leber congenital amaurosis 8; Retinitis pigmentosa 12 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_201253.3(CRB1):c.3121A>G (p.Met1041Val), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the CRB1 gene (transcript NM_201253.3) at coding-DNA position 3121, where A is replaced by G; at the protein level this means replaces methionine at residue 1041 with valine — a missense variant. Submitter rationale: For these reasons, this variant has been classified as Pathogenic. This variant disrupts the p.Met1041 amino acid residue in CRB1. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 10508521, 27380427). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C15"). This variant has been observed in individual(s) with inherited retinal dystrophy (PMID: 27113771, 29074561, 30543658). In at least one individual the data is consistent with the variant being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 636016). This variant is present in population databases (rs781705903, ExAC 0.001%). This sequence change replaces methionine with valine at codon 1041 of the CRB1 protein (p.Met1041Val). The methionine residue is highly conserved and there is a small physicochemical difference between methionine and valine.
