Pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000546.6(TP53):c.782+1G>A, citing Ambry Variant Classification Scheme 2023. This variant lies in the TP53 gene (transcript NM_000546.6) at the canonical splice donor site of the intron immediately after coding-DNA position 782, where G is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.782+1G>A pathogenic mutation results from a G to A substitution one nucleotide after coding exon 6 of the TP53 gene. This variant was determined to be de novo in at least one individual with features consistent with Li-Fraumeni syndrome (Renaux-Petel M et al. J Med Genet, 2018 Mar;55:173-180). This variant was detected in at least one individual at an allele fraction that is suggestive of clonal hematopoiesis, a predictor of TP53 pathogenicity (Ambry internal data; Fortuno C et al. Genet Med. 2022 03;24:673-680). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site; however, direct evidence is insufficient at this time (Ambry internal data). In addition to the clinical data presented in the literature, alterations that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. As such, this alteration is classified as a disease-causing mutation.
